Abstract
Primary hyperparathyroidism (PHPT) has attracted a growing attention of medical doctors of different specialties in the last few years. The ability to predict the risk of the development of visceral manifestations (nephrolithiasis, nephrocalcinosis, cardiovascular disease, peptic ulcer disease) in the mild asymptomatic PHPT can define indications for conservative treatment of such patients because of the high prevalence of the mild form of the disease. Nephrolithiasis is one of the most common manifestations of the PHPT and is one of the indications for surgery. Mechanisms of nephrolithiasis and nephrocalcinosis in PHPT remain unclear and many investigations are devoted to studying the possible predictors of its development. In this paper we review the current knowledge of nephrolithiasis, clinical features and risk factors of its development in patients with PHPT.

Abstract
We analyzed international consensuses of experts and clinical recommendations on diagnosis and treatment of Prader-Willi syndrome (PWS): PWS consensus diagnostic criteria (1993); US PWS Association (PWSA-USA) consensus statements on evaluating of breathing abnormalities (2007), osteoporosis (2008), growth hormone treatment in PWS (2000 and 2009); Endocrine society clinical practice guideline on Prevention and treatment of pediatric obesity (2008); the Second Expert Meeting of the Comprehensive Care of Patients with PWS Consensus published as Recommendations for the diagnosis and management of PWS (2008). Historical analysis and comparison of recommendations are presented in this review article. Absence of Russian clinical practice guidelines on PWS patients management makes necessary the detailed study of listed documents.

Abstract
This paper presents a case of primary hyperparathyroidism with a broad spectrum of metabolic disturbances and concomitant sleep apnea syndrome. As shown a timely surgical treatment helps to improve the mineral parameters and alleviate the risk of possible cardiovascular complications in future.

Metformin, widely used for the treatment of type 2 diabetes, is increasingly becoming the subject of research in other areas of medicine. Apart form antihyperglycemic effect of metformin has an inhibitory effect on the proliferation of various tumor cells both in vivo and in vitro. Metformin is well established in the treatment of anovulatory infertility in polycystic ovary syndrome, while its influence male reproductive function are poorly understood.

Currently, metformin is a major hypoglycemic agents for the treatment of type 2 diabetes. The main antihyperglycemic effect of metformin - suppression of hepatic gluconeogenesis and glycolysis. In rodents, brown adipose tissue (BAT) regulates cold- and diet-induced thermogenesis (CIT; DIT). Whether BAT recruitment is reversible and how it impacts on energy metabolism have not been investigated in humans. Regulatory links between BAT thermal plasticity and glucose metabolism in humans, opening avenues to harnessing BAT for metabolic benefits.

Abstract
Objectives. T study the quality of life and status of carbohydrate metabolism in patients after surgical treatment insulinoma. Methods: The study involved 20 patients divided in two groups: the first group with a catamnesis duration of up to five years; the second group with a catamnesis duration of more than five years. We studied anthropometric parameters and carbohydrate metabolism as well as psychological questioning of patients using SF-36 questionnaire, the data was considered statistically significant at p<0.05. Results. severe combined postoperative complications were more frequent in the first group (63.6% vs. 22.2%, p=0.07), due to extend of the performed surgery. Adrenergic symptoms prior to the surgery were detected in 90.9% of cases in the first group and in 77.7% of cases in the second group. After treatment these numbers decreased to 36.4% and 11.1% respectively (p=0.039 and 0.026). Neuroglycopeniс symptoms before treatment were detected in 90.9% of cases in the first group and for all patients in the second, while after treatment persisted only in 45.5% and 33.3% of cases respectively (p=0.045 and 0.036). Carbohydrate metabolism have normalized for the majority of patients. Two patients (18.2%) of the first group showed impaired glucose tolerance. Improved carbohydrate metabolism was associated with a decrease in body weight in both groups. Results of psychological questionnaires were comparable with the survey data obtained in general population in the Russian Federation. Conclusion. Surgical treatment of insulinomas is highly effective. Physical and psychological status of patients in most cases corresponds with those typical for this age-sex group of the population of the Russian Federation. Long-term treatment results do not depend on duration of the catamnesis. Complications that developed from surgical treatment have the main influence on the health of patients.

Abstract
Objectives: The aim of the study was to investigate the influence of duration and individual characteristics of sleep and chronotype on body weight, eating behavior, anxiety, depression, life quality, metabolic and hormonal parameters of obese patients. Materials and methods: 200 patients with primary obesity were studied: 83 men and 117 women at age from 18 to 61 years old, median age 41,5 years [31,0; 50,0]; body weight 107 kg [94; 128,5], waist circumference 112 cm [102; 124]; neck circumference 41 cm [38; 46], body mass index (BMI) 36,9 [32,8; 42,3]. Results: We found an association between sleep duration, chronotype and the emotional eating. Significant sleep reduction (to less than 6 hours) was associated with high level of anxiety, depression, emotional eating and insomnia. Younger age, early onset and shorter duration of obesity and brisk weight gain during last is connected to the evening chronotype. The emotional eating associated with hypersomnolence in the absence of statistically significant increase of anxiety and depression in individuals with evening chronotype. Sleep duration and chronotype have no significant effect on the body weight, metabolic, hormonal parameters and the dynamics of body. weight after 7±1 months of treatment of obesity.

Abstract
Objective: To evaluate the effectiveness and conduct a comparative analysis of consumer qualities of desmopressin intranasal drops 0.01% 5ml ("Biopharm", Georgia) in patients with central diabetes insipidus (CDI). Materials and Methods: The study included 9 patients (3 men and 6 women, mean age 44 ± 12 years) diagnosed with CDI. Patients for 7 days were switched to desmopressin intranasal drops 0.01% (JSC "Biopharm" Georgia) from there basal desmopressin preparations by means of recalculating daily dose by proposed formular: 10 mg (2 drops) of desmopressin intranasal drops = 0.2 mg of desmopressin tableted = 10 micrograms (one dose) of intranasal spray metered desmopressin = 120 mcg sublingual tablets of desmopressin. Safety biochemical parameters, efficiency to control of the main manifestations of CDI, increase in urine specific gravity, quality of life, basic consumer properties of different desmopressin preparation were evaluates at baseline and after treatment period. Results: Desmopressin nasal drops 0.01% showed to be an effective drug for the treatment of patients with CDI. Dose calculation algorithm daily was adequate in most patients. Hyponatremia was detected in 2 out of 9 patients during the study period but has been related to study drug alone in one case. The investigational drug was marked convenient to use by less than 25% of patients which may be associated with the pharmacological form of the drug as intranasal drops with special storage conditions (refrigerator). Conclusions: The drug desmopressin intranasal drops can be recommended as a second line (reserve) therapy in the treatment of patients CDI. Sodium levels should be checked during initiation of therapy.

Abstract
Objectives: To determine the prevalence of chronic fatigue syndrome (CFS) among middle-aged women and to assess the role of glucose metabolism disturbances in the development of this pathology. Materials and Methods: The study included 231 women from 40 to 60 years old (mean age 52.3 ± 5 years), observed at urban polyclinic in Moscow, who was referred to or was observed by an endocrinologist (n = 142, group 1), therapist (n = 56, group 2) or had a prophylactic medical examinations (n = 33, group 3). We recorded demographic and anthropometric data, accessed levels of glucose, glycated hemoglobin, insulin and calculated BMI and HOMA indexes. Results: The prevalence of CFS in the whole group of patients studied was 27%. The highest frequency of CFS is registered in the group of patients observed by the endocrinologist – 35%, and at dispensary examination – 21%, which was significantly different from the prevalence of CFS in patients seeking an appointment with a therapist – 13% (p = 0.002 and p = 0.03 for Fisher's exact test, respectively). Higher BMI, blood glucose, glycosylated hemoglobin, and HOMA insulin resistance index as naturally expected were observed in group 1, but after further division for the presence or absence of CFS, significant differences for patients with CFS were only higher fasting glucose levels in group 2. Conclusions: We determined the prevalence of CFS in the examined groups of patients and showed only a weak correlation of CFS and disturbances of glucose metabolism.
