Значение раннего интенсивного контроля гликемии в профилактике осложнений сахарного диабета 2 типа(результаты исследования UKPDS и последующего 10-летнего наблюдения за его участниками)

В настоящее время сахарный диабет (СД) является ведущим неинфекционным заболеванием, оказывающим значимое влияние как на качество и продолжительность жизни каждого пациента, так и являющимся тяжелым бременем для системы здравоохранения. Медико-социальная значимость сахарного диабета определяется широкой распространенностью заболевания в популяции и высоким риском развития специфических осложнений, обусловленных макро- и микрососудистыми поражениями. Согласно последним данным основным звеном развития всех специфических осложнений СД является хроническая гипергликемия, а также важную роль играют артериальная гипертензия и нарушение липидного спектра крови. В ходе многоцентрового рандомизированного контролируемого исследования UKPDS было изучено влияние уровня гликемии и артериального давления, а также основных методов их коррекции на развитие осложнений СД 2 типа. Результаты исследования UKPDS продемонстрировали значение интенсивного контроля гликемии и жесткого контроля АД для профилактики осложнений СД 2 и летальных исходов. Они позволили разработать современные рекомендации по ведению больных СД 2 и определить целевые уровни НbAlc (<7,0%) и АД (<130/80 мм). После окончания рандомизированной части исследования за его участниками проводилось 10-летнее наблюдение (10-Year Follow-up of Intensive Glucose Control in Type 2 Diabetes) с целью определить оказывает ли проведенная терапия долгосрочный эффект на осложнения СД 2 типа. Результаты этого наблюдения продемонстрировали сохранение благоприятного влияния интенсивной сахароснижающей терапии на риск осложнений СД и летальных исходов
