Одним из последствий ожирения для женского организма является снижение фертильности вплоть до бесплодия. Показано, что нарушение репродуктивной функции у больных ожирением может быть связано, в частности, со снижением овариального резерва.</p></sec><sec><title>Цель</title><p>Цель. Сравнить овариальный резерв у больных ожирением различной степени и женщин без ожирения репродуктивного возраста.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследовано 320 женщин молодого репродуктивного возраста (от 20 до 30 лет). Участницы были разделены на 4 равные группы (n=80) в зависимости от индекса массы тела (ИМТ): участницы без ожирения (ИМТ&lt;30 кг/м2) и больные ожирением I–III степени в соответствии с критериями Всемирной организации здравоохранения. У всех больных на 2–3-й день менструального цикла определяли следующие маркеры овариального резерва: сывороточные уровни антимюллерова гормона (АМГ), ингибина В, фолликулостимулирующего гормона (ФСГ), лютеинизирующего гормона (ЛГ), эстрадиола, тестостерона и прогестерона, а также ультразвуковые параметры – количество антральных фолликулов и объем яичников.</p></sec><sec><title>Результаты</title><p>Результаты. Статистически достоверно при сравнении параметров у женщин без ожирения и у больных ожирением различались уровень АМГ, тестостерона, объем яичников, а также количество антральных фолликулов. При этом по всем параметрам выявлены достоверные различия у женщин с разной степенью ожирения. В то же время даже у больных ожирением III степени параметры не выходили за референсные пределы.</p></sec><sec><title>Заключение</title><p>Заключение. Для больных ожирением женщин молодого репродуктивного возраста характерно прогрессивное снижение показателей овариального резерва с увеличением степени ожирения, однако значения оцениваемых параметров даже у больных ожирением III степени остаются в пределах референсных значений. Для уточнения роли ожирения в снижении овариального резерва необходимо проведение крупных рандомизированных многоцентровых исследований с включением пациенток различных рас с оценкой влияния сопутствующих факторов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: One of the consequences of obesity for the female body is a decrease in fertility. It is shown that impaired reproductive function in obese patients can be associated, in particular, with a decrease in ovarian reserve.</p></sec><sec><title>AIMS</title><p>AIMS: To evaluate the ovarian reserve function in female patients of reproductive age with different classes of obesity in comparison with women without obesity.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: This study evaluated 320 caucasian women, age 20-30 years, without obesity (BMI&lt;30, n=80) and with obesity WHO class I-III (n=80 per class). Anthropometrics, serum concentrations of anti-Mullerian hormone (AMH), inhibin B, follicle stimulating hormone (FSH), luteinizing hormone (LH), estradiol, progesterone, and testosterone were compared on the 2-3 day of menstrual cycle as ovarian volume and antral follicle count (AFC).</p></sec><sec><title>RESULTS</title><p>RESULTS: We reveal statistically significant difference in following parameters in normal BMI women in comparison with obesity women: AMH, testosterone, ovarian volume and AFC. Moreover, we reveal significant difference between patients with different WHO class of obesity. But even in class III obesity parameters remained within reference ranges.</p></sec><sec><title>CONCLUSIONS</title><p>CONCLUSIONS: Ovarian reserve function parameters progressively decrease with increase of obesity class in subjects, but ovarian reserve parameters were in normal reference range even in class III obese patients.
