Ожирение – важнейший фактор кардиометаболического риска. Однако у ряда пациентов нарушения липидного, углеводного обмена и кардиоваскулярная патология отсутствуют – метаболически здоровое ожирение (МЗО).</p></sec><sec><title>Цель работы</title><p>Цель работы. Определение распространенности МЗО, изучение особенностей данного фенотипа в сравнении с метаболическим синдромом (МС).</p></sec><sec><title>Методы</title><p>Методы. Для оценки распространенности МЗО проанализировано 389 историй болезни пациенток 18–60 лет с ожирением. Использовано три варианта критериев МЗО: 1) с учетом индекса НОМА (&lt;2,7); 2) с учетом дефиниций МС от 2005 г.; 3) критерии BioSHaRE-EU от 2013 г. (пациенты с ожирением без единого проявления МС). Проведен сравнительный анализ данных анамнеза, антропометрии, состояния липидного и углеводного обменов, функционального состояния печени.</p></sec><sec><title>Результаты</title><p>Результаты. С учетом индекса НОМА распространенность МЗО составила 34,5%, с учетом дефиниций МС от 2005 г. – 38,6%, по BioSHaRE-EU – 9,6%. В группах МЗО и МС дислипидемия отмечалась в 27,3 и 49,5% (p&lt;0,05), артериальная гипертензия – в 25% и 71,6% (p&lt;0,05), стеатогепатоз – в 47,7% и 51,3% (p&lt;0,05) наблюдений соответственно. Среди коморбидностей преобладала гинекологическая патология: у 50,8 и 61,4% (р&lt;0,05), существенно отличалась частота различных нарушений углеводного обмена: 6,8 и 39,1% случаев (р&lt;0,05). На момент обследования пациентки с МЗО имели меньшую длительность существования ожирения (18,7 и 24 года) (р=0,0004) и реже предпринимали попытки снизить вес – 85,8% и 91,6%. Средние значения ИМТ, окружности талии, окружности бедер, глюкозы натощак, общего холестерина, инсулина базального, С-пептида базального, индекса НОМА, в группах МЗО и МС достоверно различались (p&lt;0,05). Медиана АЛТ составила 20 и 23,2 ед/л, АСТ – 20 и 23 ед/л, триглицеридов – 1,1 и 1,8 ммоль/л, липопротеидов высокой плотности – 1,4 и 1,1 ммоль/л соответственно.</p></sec><sec><title>Заключение</title><p>Заключение. Максимальная распространенность МЗО определена с учетом критериев МС от 2005 г., минимальная – по критериям BioSHaRE-EU. Основные проанализированные показатели достоверно различались в группах МЗО и МС. Большая длительность существования ожирения в группе МС предполагает нестабильность фенотипа МЗО с течением времени.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Obesity is a major factor for cardiometabolic risk. However, there is a category of obese patients without disorders of lipid, carbohydrate metabolism and cardiovascular disease – metabolically healthy obese (MHO).</p></sec><sec><title>Aim</title><p>Aim. Our goal was to investigate the prevalence and characteristics of this phenotype compared to patients with metabolic syndrome (MS).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. To evaluate the prevalence of the MHO phenotype we analyzed 389 medical records of females aged 18–60 years with obesity. Three types of MHO criteria were used: 1) HOMA index (&lt;2.7); 2) IDF-criteria of metabolic syndrome, 2005; 3) the BioSHaRE-EU 2013 criteria (obese patients without any symptoms of MS). We conducted a comparative analysis of anthropometry, status of lipid and carbohydrate metabolism, the functional state of the liver.</p></sec><sec><title>Results</title><p>Results. The MHO prevalence was: 34.5% according to HOMA index, according to the definitions of MS 2005 – 38.6%, in BioSHaRE-EU – 9.6%. In groups of MHO and MS dyslipidemia was observed in 27.3 and 49.5% (p&lt;0.05), hypertension in 25% and 71.6% (p&lt;0.05), steatogepatosis in 47.7% vs 51.3% (p&lt;0.05) of observations, respectively. Among comorbidities the gynecological pathology was most prevalent - 50.8 and 61.4% (p&lt;0.05), disorders of carbohydrate metabolism differed significantly in frequency- 6.82 and 39.1% of patients (p&lt;0.05). Patients with MHO had a shorter duration of the existence of obesity than MS (18.7 vs. 24 years) (p=0.0004) and less likely to have attempted to reduce weight – 85.8% and 91.6%. Average BMI, waist circumference, hip circumference, fasting glucose, total cholesterol, insulin basal, basal C-peptide, HOMA index in groups of MHO and MS differed significantly (p&lt;0.05). Median ALT was 20 and 23.2 U/l, AST – 20 and 23 U/l, triglycerides – 1.1 and 1.8 mmol/l, high-density lipoprotein – 1.4 and 1.1 mmol/l, respectively.</p></sec><sec><title>Conclusions</title><p>Conclusions. The MHO prevalence was maximal according to the MS definitions from 2005, and minimal with BioSHaRE-EU criteria. The main analyzed indicators differed significantly in groups MHO and MS.
