В зависимости от причины возникновения этого заболевания, выделяют первичный (ПГПТ), вторичный (ВГПТ) и третичный гиперпаратиреоз (ТГПТ). При ВГПТ уменьшение количества кальцийчувствительных (CaSR) и витамин-D-чувствительных (VDR) рецепторов приводит к нарушению фосфорно-кальциевого обмена и развитию гиперплазии ОЩЖ. Лечение ВГПТ витамином D показало свою эффективность не только в нормализации клинико-лабораторных показателей, но и в обратном развитии изменений в ОЩЖ. В данной статье представляется разбор клинического случая пациентки с диагнозом вторичного гиперпаратиреоза на фоне дефицита витамина D, объемным образованием околощитовидной железы более 1 см в диаметре, с положительной динамикой уровня ПТГ, а также полной редукцией образования ОЩЖ на фоне длительной пероральной терапии активными метаболитами витамина D.</p></abstract><trans-abstract xml:lang="en"><p>Hyperparathyroidism is a disease characterized by excessive secretion of parathyroid hormone (PTH) in the chief cells of parathyroid glands. There are three types of hyperparathyroidism: primary, secondary, and tertiary depending on the cause of this disease. By the secondary hyperparathyroidism calcium-sensing receptors (CaSR) and vitamin D receptors (VDR) lead to disturbance of phosphorus-calcium exchange and to development of a parathyroid glands hyperplasia. Treatment of secondary hyperparathyroidism with vitamin D showed the efficiency not only in normalization of clinical laboratory indicators but also in involution of changes in parathyroid glands. We represent the clinical case of a patient with secondary hyperparathyroidism caused by hypovitaminosis D, parathyroid hyperplasia more than 1 cm.
