Пациенты с СД имеют повышенный риск перелома костей. У лиц с сахарным диабетом 1 типа (СД1) он возрастает более чем в 6 раз. Существует несколько механизмов, приводящих к развитию остеопороза при СД1: хроническая гипергликемия, дефицит инсулина, генетические факторы, осложнения СД1. Хроническая болезнь почек при СД1 оказывает влияние не только на прогрессирование остеопороза, но и приводит к возникновению других костных нарушений, что значительно осложняет выбор лечебной тактики. В данной работе приводится описание клинического случая минеральных и костных нарушений, развившихся у пациентки с длительным анамнезом СД1, получающей заместительную почечную терапию программным гемодиализом.</p></abstract><trans-abstract xml:lang="en"><p>Osteoporosis and diabetes mellitus (DM) – chronic diseases with constantly growing prevalence. Patients with DM have the increased risk of bones fractures. Risk of fracture in the patient with diabetes mellitus type 1 (DM1) is increased by more than 6 times. There are several mechanisms leading to the development of osteoporosis in DM: chronic hyperglycemia, insulin deficiency, genetic factors, and complications of DM. Moreover, the chronic kidney disease in DM impacts not only progression of osteoporosis, but also leads to emergence of other bone disorders, that considerably complicate a choice of antiosteoporotic treatment.
