Атеросклеротическое поражение магистральных артерий головы, в том числе с наличием атеросклеротических бляшек и гемодинамически значимых стенозов (≥60%), превалировало у пациентов с СД2. При сопоставимых объемах поражения головного мозга и исходном уровне неврологического дефицита на фоне СД2 чаще отмечается нарастание неврологической симптоматики, что нередко обусловливает худший функциональный исход. Обнаружено, что менее благоприятный прогноз ОНМК ассоциирован не с транзиторными изменениями уровня глюкозы плазмы, а с развитием СД2. У большинства пациентов с СД2 развитие ОНМК происходило на фоне повышения не только гликемии, но и гликированного гемоглобина (HbA1c), что свидетельствует о том, что сосудистой катастрофе предшествовал длительный период гипергликемии.</p></abstract><trans-abstract xml:lang="en"><p>We carried out a comparative analysis of the clinical course of acute stroke in patients with and without type 2 diabetes mellitus (T2DM). The prevalence of carotid atherosclerosis (incl. those with plaques and hemodynamically significant [≥60%] carotid stenosis) was higher in patients with T2DM. With relatively similar baseline brain damage and neurological deficits patients with T2DM had a higher rate of symptom worsening, which leads to poorer outcome. A less favourable post-stroke prognosis was found to be associated with the presence of T2DM, but not with transient changes in glycemic levels.
