Синдром поликистозных яичников (СПЯ) - заболевание, характеризующееся хронической ановуляцией, наличием
клинических или биохимических проявлений гиперандрогении и мелкокитозными изменениями яичников. Одним из наи-
более значимых этапов в изучении патогенеза СПЯ стало открытие взаимосвязи между овариальной гиперандрогенией и нарушением чувствительности к инсулину. С целью сравнительного изучения влияния инсулинорезистентности на стероидную
активность цитохрома Р450с17α авторы провели динамическое, активное, проспективное исследование 60 женщин раннего
репродуктивного возраста, страдающих СПЯ, средний возраст которых составил 25,2±5,8 (М-25,0). Пациенткам были проведены функциональные пробы с блокатором гонадолиберина (ГнРГ) и адренокортикотропным гормоном (АКТГ). После
этого в течение 3 мес. проводилась терапия сенситайзером к инсулину. В результате проведенного лечения при повторном
проведении проб изучалась динамика некоторых показателей овариального и надпочечникового стероидогенеза. Авторы делают выводы, что компенсаторная гиперинсулинемия и ИР являются причиной гиперандрогении, лечение метформином
приводит к значительному снижению продукции андрогенов яичниками, в том числе и за счет снижения ЛГ-зависимого стероидогенеза в овариальной ткани, а значительное снижение уровня 17-ОН-Р у больных с СПЯ и ИР на фоне 3 мес. лечения
метформином может свидетельствовать о воздействии препарата на ферментативную активность Р450с17α.
Resume. Polycystic ovary syndrome (PCOS) - the disease characterised by chronic anovulation, presence of clinical or biochemical
manifestations of hyperandrogenism and polycystic ovary changes. One of the most significant stages in studying pathogenesis of
PCOS was the discovery of the interrelation between ovarian hyperandrogenism and decreased sensitivity to insulin. For the purpose
of comparative studying of influence of the insulin resistance on steroid activity of cytochrome Р450с17α authors undertook a dynamic,
active, prospective research in 60 women of the early reproductive age with PCOS, with middle age 25,2+5,8 (М-25,0). In this group
of patients we performed functional tests with a blocker of gonadotropin-releasing hormone and adrenocorticotropic hormone analogfollowed
by 3 months therapy with insulin sensitizer (metformin) with repeat of the functional tests at the end of treatment period to
assess the indicators of ovarian and adrenal sex hormones secretion. Authors conclude that hyperinsulinemia and insulin resistance
are the cause of hyperandrogenism, and treatment with insulin sensitizers leads to considerable decrease in production of ovarian androgens
including at the expense of decrease luteinizing hormone-dependent sex steroid sinthesis in ovarian tissue, and considerable
decrease in level 17-hydroxyprogesterone in patients with PCOS and insulin resistance after 3 month treatment with insulin sensitizer
