В данном открытом проспективном исследовании принимали участие 157 пациентов с СД2, которые были переведены на терапию инсулином: 112 женщин и 45 мужчин, средний возраст 57 лет, средняя продолжительность заболевания 10 лет. Были сформированы 2 группы наблюдения. Первую группу составили 78 человек в возрасте 57 лет [45; 73] с длительностью заболевания 10 лет [4; 16], в лечении которых были использованы аналоги инсулина – базальный инсулин (гларгин, детемир), двухфазный инсулин (Аспарт 30, Хумалог Микс 25) и инсулин короткого действия (лизпро, аспарт). Во вторую группу вошли 79 пациентов в возрасте 59 лет [46; 75] со стажем заболевания 10 лет [5; 15], которые получали лечение препаратами инсулина человека продолжительного (Протафан, Хумулин НПХ) и короткого действия (Актрапид, Хумулин Р), а также двухфазным инсулином (Микстард, Хумулин М3). Исходно и через год наблюдения у больных определяли параметры углеводного обмена (HbA1c, базальную и постпрандиальную гликемию) и содержание липидов в сыворотке крови (ОХС, ТГ, ХС ЛПНП, ХСЛПВП); а также контролировали рост, вес и окружность талии. В ходе исследования было выявлено, что терапия аналогами инсулина обеспечивает статистически значимое снижение уровня HbA1c, базальной и постпрандиальной гликемии, а также увеличение частоты достижения целевых уровней этих показателей по сравнению с терапией препаратами инсулина человека. Терапия аналогами инсулина сопряжена с меньшим риском увеличения окружности талии у больных СД2 в течение первого года терапии инсулином по сравнению с препаратами инсулина человека. Применение аналогов инсулина позволило достичь статистически значимо более низких уровней ОХС, ТГ, ХС ЛПНП, ХС ЛПВП по сравнению с использованием человеческих инсулинов. Увеличение массы тела у пациентов с СД2, получающих терапию инсулином, не оказывает отрицательного влияния на факторы риска сердечно-сосудистых заболеваний, в частности, на липидный спектр крови.</p></abstract><trans-abstract xml:lang="en"><p>The aim of the present study was to investigate whether insulin treatment-induced weight gain had an adverse impact on cardiovascular risk factors in insulin-treated type 2 diabetic patients during the first year after initiating insulin therapy when insulin analogues or human insulins are used. A total of 157 patients with newly insulinized type 2 diabetes were included in the study. The patients were divided in two groups. First group consisted of subjects (mean age 57 [45; 73], duration of diabetes of 10 years [4; 16]) who had received long-acting basal (glargine, detemir), premixed (biphasic insulin aspart 30, Humalog Mix 25) or short-acting (aspart, lispro) insulin analogues. Patients from second group (mean age 59 [46; 75], duration of diabetes of 10 years [5; 15]) were treated with intermediate-acting basal (Protophane, Humulin NPH insulin), premixed (biphasic human insulin 30, Humulin M3) and regular (Actrapid, Humulin R) human insulins. Our study has shown that insulin-induced weight gain may not adversely affect cardiovascular risk factors, particularly, lipid profile, in insulin-treated type 2 diabetic patients during the first year after initiating insulin therapy.
