Материалы и методы: в исследование было включено 60 молодых мужчин с ожирением (индекс массы тела (ИМТ)) &gt;30 кг/м2 в возрасте от 16 до 25 лет. Пациенты первой группы (n=30) получали медикаментозное лечение орлистатом (по 120 мг 3 раза в день с основным приемом пищи), на протяжении 12 недель. Пациенты второй группы (n=30) получали немедикаментозное лечение - диета в сочетании с физическими нагрузками. На скрининге и заключительном визите всем участникам исследования проводилось биохимическое исследование и анализ крови на гормоны, 48 участников были обследованы психологическими методиками (опросники депрессии и тревоги Бека, шкалы социальной тревоги Либовица и Шихана, а также опросник пищевого поведения DEBQ). Результаты: на фоне лечения орлистатом (Ксеникал) большему количеству пациентов удалось добиться клинически значимого снижения веса: у 50% (15 пациентов) больных масса тела (МТ) снизилась более чем на 5%, у 30,0% (9 пациентов) - более чем на 10% от исходной МТ. Только у 20,0% (6 пациентов) больных МТ снизилась менее чем на 5% от исходной. Также было отмечено снижение уровня кортизола и повышение уровня тестостерона. В контрольной группе пациентов эффективность лечения оказалась менее значимой. Отмечено достоверное снижение показателей тревоги и депрессии в группе пациентов, принимавших орлистат (p&lt;0,05). Высокие показатели социальной тревоги не снизились в обеих группах. В результате терапии орлистатом отмечалось снижение экстернального и увеличение выраженности ограничительного пищевого поведения. Выводы. По результатам данного исследования терапия орлистатом приводила к снижению веса, что сопровождалось улучшением гормональных и биохимических показателей, а также изменением показателей тревоги и депрессии.</p></abstract><trans-abstract xml:lang="en"><p>Objective: to assess hormonal and metabolic parameters and psychological status of young men with obesity. Methods: The study included 60 men with obesity (BMI&gt;30 kg/m2) divided in two groups. Patients in the first group (n=30) received orlistat for 12 weeks (120 mg 3 times daily with meal). Patients in second group (n=30) followed hypocaloric diet and aerobic exercise. All patients were examined before treatment and after 12 weeks. Evaluation included hormonal and biochemical analyses, 48 patients were examined by psychological questionnaires (Beck Depression Inventory, Beck Anxiety Inventory, Liebowitz Social Anxiety Scale, Dutch Eating Behavior Questionnaire). Results: Patients that received orlistat treatment showed significant decrease of body mass: 50% of patients had decrease more than 5%, 30% of patients - more than 10% (p&lt;0,05). In first group after 12 weeks of treatment level of cortisol decreased and level of testosterone increased. The results of treatment in second group were less significant. There was a significant decrease in anxiety and depression scales in patients taking orlistat (p&lt;0,05). High levels of social anxiety did not decrease in both groups after treatment. As a result of orlistat treatment there was a decrease in external eating behavior and increase in expression of restraint eating behavior by DEBQ (p&lt;0,05). Conclusions: treatment with orlistat reduces body weight, which is correlated by improvement of hormonal and biochemical parameters.
