для коррекции прокоагулянтных и провоспалительных нарушений, ассоциированных с метаболическим синдромом в постменопаузе. Материалы и методы: в проспективное, открытое, рандомизированное исследование длительностью 12 месяцев
было включено 58 женщин (возраст 55 [53; 56] лет; постменопауза длительностью 5 [4; 8] лет) c метаболическим синдромом
(IDF, 2005). Участники исследования были рандомизированы на две группы - комбинированной (метформин в дозе 1 г
в сутки + модификация образа жизни) (n=29) и немедикаментозной (контрольная группа) (n=29) терапии. Забор крови производился исходно и по завершении лечения для определения параметров свертывающей и противосвертывающей систем
крови: протромбинового индекса, активированного частичного тромбопластинового времени (АЧТВ), тромбинового времени, активностей факторов VII (FVII), VIII (FVIII), фактора Виллебранда (vWF), антитромбина III, ингибитора активатора
плазминогена-1 (ИАП-1), активатора плазминогена тканевого типа (t-PA), протеин С-скрининг тест. Также проводилось
определение концентраций фибриногена, С-реактивного белка (СРБ) и адипонектина в крови. Результаты: по завершению
1 года терапии в группе метформина отмечалось значимое снижение уровня СРБ с 4,98 [2,72; 11,95] до 2,56 [1,92; 6,76] мг/л
(р&lt;0,0001), улучшение фибринолиза (активность ИАП-1 в плазме крови снизилась с 21,1 [8,86; 33,59] до 9,5 [4,69; 16,14]
Ед/мл (p=0,0005)), а также повышение концентрации адипонектина с 14,36 [7,71; 17,4] мкг/мл до 10,89 [6,84; 16,78] мкг/мл
(р=0,0013). Данные изменения были независимы от улучшения чувствительности к инсулину (снижения индекса НОМА-IR)
на фоне приема метформина. Также отмечалось умеренное снижение активности FVII, концентрации фибриногена в плазме
крови и повышение активности антикоагулянтной системы протеина С, однако эти изменения оказались статистически
незначимыми. В контрольной группе каких-либо значимых улучшений исследуемых параметров выявлено не было. Вывод:
комбинированная терапия, включающая прием метформина, в отличие от немедикаментозного лечения, приводит к кор-
рекции ассоциированных с метаболическим синдромом провоспалительных сдвигов (концентраций СРБ и адипонектина)
и недостаточности фибринолиза (повышения активности ИАП-1).</p></abstract><trans-abstract xml:lang="en"><p>The aim of the study was to assess the effect of metformin plus lifestyle modification on procoagulant and proinflammatory state
associated with metabolic syndrome in postmenopausal women. Materials and methods: The 12-month prospective, open, randomized
study included 58 postmenopausal women (age 55 [53; 56]; postmenopause 5 [4; 8] years) with metabolic syndrome (IDF, 2005).
Participants were randomized into two groups receiving either metformin (1 g per day) plus lifestyle modification (n=29), or lifestyle
modification alone (control group) (n=29). Blood tests were performed before and after the treatment period to evaluate parameters
of coagulant and anticoagulant blood systems: prothrombin index, activated partial tromboplastin time (APTT), thrombin time, activities
of factors VII (FVII),VIII (FVIII), von Willebrand factor (vWF), antithrombin III, plasminogen activator inhibitor-1 (PAI-1),
tissue-type plasminogen activator (t-PA) and screening test for the protein C pathway. Also we assessed fibrinogen, C-reactive protein
(CRP) and adiponectin concentrations. Results: After one year of treatment in metformin group CRP level decreased significantly
from 4,98 [2,72; 11,95] to 2,56 [1,92; 6,76] mg/l (р&lt;0,0001), fibronolysis improved (PAI-1 activity decreased from 21,1 [8,86; 33,59]
to 9,5 [4,69; 16,14] U/ml (p=0,0005)) and adiponectin level increased from 10,89 [6,84; 16,78] to 14,36 [7,71; 17,4] mcg/ml (р=0,0013).
All these changes were independent from improvements in insulin sensitivity (HOMA-IR reduction) as a result of metformin use.
Also we observed moderate decreases in FVII activity, fibrinogen concentration and increase in protein C anticoagulant pathway,
but these changes were not significant. In control group there were no any significant changes in observable parameters. Conclusion:
Metformin plus lifestyle modification, in contrast to lifestyle modification alone, resulted in improvements of proinflammatory state
