В работе представлены результаты 5-летних наблюдений за 51 пациентом, перенесшим операцию билиопанкреатического шунтирования (БПШ) с выключением двенадцатиперстной кишки (BPD/DS). Возраст пациентов составлял 42,9±11,1 лет, предоперационное значение ИМТ - 47±6,1 кг/м2. Двенадцать пациентов ранее перенесли неудачные бариатрические операции (11 операций бандажирования желудка, 1 - вертикальная гастропластика). Наблюдение проводилось каждые 3 мес на протяжении первого года после операции, каждые 6 мес - на протяжении второго года и затем ежегодно. Долгосрочное наблюдение за 7 пациентами (7,8%) оказалось невозможным по следующим причинам: 2 пациента скончались спустя 9 месяцев после БПШ (1 инфаркт миокарда и 1 послеоперационный перитонит после операции по поводу грыжи брюшной стенки), 1 случай восстановления пищеварительного тракта, 1 случай отказа от обследования после сложного течения послеоперационного периода, утеря контакта с 3 пациентами. Через пять лет после операции среднее значение ИМТ составило 31±4,5 кг/м2 с потерей избыточного веса, равной 71,9±20,6%. У семи из 44 пациентов отмечена потеря избыточного веса менее 50% от дооперационной избыточной массы тела (15,9%), причем у 4 из них уже была отмечена неудача после первой бариатрической операции. После первичных операций BPD/DS потеря избыточного веса составила 75,8±18,0%. Заключение. БПШ позволяет достигать устойчивой и значительной потери избыточного веса в сроки свыше 5 лет после операции, причем неудовлетворительные результаты отмечены менее чем в 20% случаев.
