Изучить особенности секреции инсулина, PYY и ГПП-1 после приема эритритола в сравнении с инкретиновым ответом после приема сахарозы и совместного приема эритритола и сахарозы у пациентов с ожирением.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включены пациенты 18–35 лет с ожирением I–II степени (ИМТ 30–40 кг/м²) без нарушений углеводного обмена. Каждый испытуемый получил 75 г сахарозы, 75 г эритритола или комбинацию сахарозы (75 г) с эритритолом (25 г) в виде раствора (3 теста проводились последовательно, с временным интервалом между тестами не менее 48 часов). Концентрации глюкозы, инсулина, PYY и ГПП-1 определялись в базальной точке и через 30, 60, 90 и 120 минут после приема раствора. Методика полностью повторяла протокол предыдущего исследования, проведенного на здоровых добровольцах, что обеспечило возможность сопоставить результаты.</p></sec><sec><title>Результаты</title><p>Результаты. Прием эритритола не вызывал повышения уровня глюкозы крови. Уровень инсулина после приема эритритола оставался неизменным. Максимальная концентрация глюкозы после употребления сахарозы составила 7,55 [6,59; 8,07] ммоль/л, после приема смеси (эритритол + сахароза) — 7,20 [7,05; 7,66] ммоль/л (р=0,598). Повышения PYY не зафиксировано ни в одной из проб. Концентрация ГПП-1 на 120-й минуте была достоверно выше после приема эритритола по сравнению с сахарозой (р=0,0017).</p></sec><sec><title>Заключение</title><p>Заключение. У пациентов с ожирением прием эритритола не сопровождается повышением уровня глюкозы и инсулина, но вызывает увеличение секреции ГПП-1, что может способствовать усилению чувства насыщения и замедлению опорожнения желудка. Эритритол может рассматриваться как безопасный и физиологически оправданный сахарозаменитель у лиц с ожирением.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>OBJECTIVE</title><p>OBJECTIVE. To investigate postprandial secretion of insulin, peptide YY (PYY), and glucagon-like peptide-1 (GLP-1) following erythritol ingestion in patients with obesity, and to compare these responses with hormonal changes after sucrose intake and combined erythritol–sucrose administration.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS. Adults aged 18–35 years with class I–II obesity (BMI 30–40 kg/m²) and without carbohydrate metabolism disorders were enrolled. Each participant completed three oral tests on separate days, receiving solutions containing either 75 g sucrose, 75 g erythritol, or 75 g sucrose plus 25 g erythritol, with washout intervals ≥48 hours. Plasma glucose, insulin, PYY, and GLP-1 were measured at baseline and at 30-, 60-, 90-, and 120-minutes post-ingestion. The protocol duplicated our prior study in healthy volunteers, enabling direct comparison of metabolic and incretin responses between the two populations.</p></sec><sec><title>RESULTS</title><p>RESULTS. Erythritol ingestion did not increase plasma glucose or insulin concentrations. Peak glucose after sucrose intake reached 7.55 [6.59; 8.07] mmol/L, compared with 7.20 [7.05; 7.66] mmol/L after the erythritol–sucrose mixture (p = 0.598). No increase in PYY was observed under any test condition. GLP-1 levels at 120 minutes were significantly higher after erythritol ingestion compared with sucrose (p=0.0017).</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION. In patients with obesity, erythritol does not elevate plasma glucose or insulin levels but enhances GLP-1 secretion, which may contribute to improved satiety signaling and delayed gastric emptying.
