Выявление характера изменений состава тела и взаимосвязи с клинической картиной ХОБЛ является актуальной проблемой, так как полученные данные могут быть использованы для оценки исхода и прогноза заболевания.</p></sec><sec><title>Цель</title><p>Цель. Установить взаимосвязь компонентного состава тела с особенностями клинического течения и функции внешнего дыхания пациентов с хронической обструктивной болезнью легких.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Исследование проводилось в клиниках Сибирского государственного медицинского университета, в том числе были обследованы приглашенные пациенты из пульмонологического отделения ОГАУЗ «Городская клиническая больница №3 им. Б.И. Альперовича». Данное исследование многоцентровое, интервенционное, двухвыборочное, сравнительное. Были включены пациенты с ХОБЛ и условно-здоровые лица для группы контроля. Всем были проведены антропометрия, анкетирование, биоимпедансометрия, спирометрия и проба на обратимость бронхообструкции.</p></sec><sec><title>Результаты</title><p>Результаты. Пациенты с ХОБЛ в количестве 105 человек были условно поделены на три группы по ИМТ. Группа контроля составила 40 человек. Распределение больных ХОБЛ по спирометрической классификации GOLD при различной массе тела отличалось статистически значимо (χ² =13,81; p=0,031). Наибольшее количество пациентов с GOLD IV наблюдалось в группе с нормальной массой тела. У больных ХОБЛ с ожирением и избыточной массой тела выше показатели тощей массы тела и висцерального жира. Пациенты со стадией GOLD I имели наибольшее количество жидкости в организме, минеральной массы, висцерального жира, а также тощей и мышечной масс тела, а со спирометрической стадией GOLD IV характеризовались наименьшими показателями мышечной массы, жировой ткани. Обнаружено, что при большем количестве жировой ткани наблюдаются лучшие показатели функции внешнего дыхания. Висцеральный жир имел положительную корреляцию с количеством баллов опросников CAT и CCQ. Взаимосвязи между составом тела и количеством обострений в год обнаружено не было.</p></sec><sec><title>Заключение</title><p>Заключение. Показатели спирометрии у больных ХОБЛ имеют взаимосвязь не только с мышечной и жировой тканью, а также с количеством жидкости в организме и минеральной массой тела. Больные ХОБЛ с нормальной массой тела имели наименьшее количество жировой ткани и наихудший показатель ОФВ1. Пациенты со стадией GOLD I имели наибольшее количество жидкости в организме, минеральной массы, висцерального жира, а также тощей и мышечной масс тела, а при GOLD IV регистрировалось наибольшее количество пациентов с дефицитом мышечной массы тела и избытком жировой ткани. Это подчеркивает важность оценки состава тела в клиническом ведении пациентов с ХОБЛ.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Identifying the nature of changes in body composition and the relationship with the clinical course of COPD is an urgent problem, since the data obtained can be used to assess the outcome and prognosis of the disease.</p></sec><sec><title>AIM</title><p>AIM: To establish the relationship between body composition and the characteristics of the clinical course of COPD.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: The study was conducted in the Clinics of the Siberian State Medical University, including the examination of invited patients from the City Clinical Hospital No. 3. This study is multicenter, interventional, two-sample, comparative. Patients with COPD and apparently healthy controls were included. All of them underwent anthropometry, questionnaires, bioimpedance measurements, spirometry and a test for the reversibility of bronchial obstruction.</p></sec><sec><title>RESULTS</title><p>RESULTS: 105 patients with COPD were conditionally divided into three groups according to BMI. The control group consisted of 40 people. The greatest number of patients with GOLD IV was observed in the group with normal body weight. Obese and overweight COPD patients have higher levels of lean body mass and visceral fat. Patients with GOLD stage I had the highest amounts of body fluid, mineral mass, visceral fat, and lean and lean body mas, and with stage GOLD IV had the lowest levels of muscle mass and adipose tissue. It was found that with a larger amount of adipose tissue, better indicators of external respiration function are observed. Visceral fat was positively correlated with CAT and CCQ scores.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: Spirometry indicators in patients with COPD are related not only to muscle and fat tissue, but also to the amount of fluid in the body and mineral mass of the body. COPD patients with normal body weight had the least amount of adipose tissue and the worst FEV1. Patients with stage GOLD I had the highest amount of body fluid, mineral mass, visceral fat, as well as lean and lean body mass, while GOLD IV had the highest number of patients with a deficiency of lean body mass and excess adipose tissue.
