Как вегетативный дисбаланс контроля сердечного ритма, оцениваемый по вариабельности сердечного ритма (ВСР), так и признаки резистентности к инсулину достаточно распространены в выборке мужчин-северян. Однако до настоящего времени нет данных, связаны ли отклонения в ВСР в более старших возрастных группах жителей Севера с наличием инсулинорезистентности.</p></sec><sec><title>Цель</title><p>Цель. Проведение сравнительного анализа основных характеристик ВСР у мужчин зрелого возраста, проживающих в условиях Российского Севера, с учетом наличия или отсутствия метаболических признаков инсулинорезистентности.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Обследованы 73 мужчины зрелого возраста — постоянные жители Магаданской области из числа европеоидов. Анализируемая группа была дифференцирована на выборку без признаков (индекс HOMA-IR&lt;2,5 усл. ед.) и с признаками инсулинорезистентности (индекс HOMA-IR&gt;2,5 усл. ед.). В работе использовали иммунохемилюминесцентный и ферментативный методы, а также метод оценки ВСР c помощью прибора «Варикард» (Россия).</p></sec><sec><title>Результаты</title><p>Результаты. Показано, что у 48% пациентов в анализируемой выборке были зафиксированы признаки инсулинорезистентности, что наблюдалось на фоне увеличения симпатической активности в регуляции сердечного ритма у представителей данной группы. Проведенный анализ основных характеристик кардиоритма у мужчин, различающихся по величине индекса HOMA-IR, позволил выявить ряд показателей, претерпевающих наиболее существенные изменения в зависимости от наличия или отсутствия признаков инсулинорезистентности: MxDMn, pNN50, SDNN, AMo50, SI, TP, HF, LF, масса тела.</p></sec><sec><title>Заключение</title><p>Заключение. В целом полученные нами результаты свидетельствуют о достаточно высокой доле лиц с наличием признаков инсулинорезистентности в группе мужчин-северян зрелого возраста. Полученные результаты указывают на то, что обследуемые мужчины с признаками инсулинорезистентности характеризуются наличием вегетативного дисбаланса с относительным доминированием симпатической активности на фоне снижения активации парасимпатического звена вегетативной нервной системы и большими величинами массы тела. При этом взаимоотношения и причинно-следственные связи между признаками инсулинорезистентности, активации симпатического звена вегетативной нервной системы и избыточной массы тела во многом остаются невыясненными. По-видимому, все анализируемые признаки, скорее всего, взаимно дополняют, чем полностью исключают друг друга, при этом триада в виде ожирения, признаков инсулинорезистентности и активации симпатического звена вегетативной нервной системы является предпосылкой для значительных рисков нарушения здоровья. Необходимо отметить, что данное исследование направлено на расширение использования метода оценки вариабельности сердечного ритма с учетом наличия или отсутствия метаболических признаков инсулинорезистентности в выборке мужчин зрелого возраста.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Heart rate autonomic regulation can go out of balance which is normally assessed by the heart rate variability (HRV) indices. Similarly, it is relevant to research if and how HRV fluctuations can be influenced by varying signs of insulin resistance since they are quite common in Northern men. At present, there is no evidence of this influence in the North residents of older ages.</p></sec><sec><title>AIM</title><p>AIM: This study aimed to comparatively assess heart rate variability in mature men who do or do not feature metabolic signs of insulin resistance.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: Seventy-three mature aged male residents of Magadan Region, Caucasian by origin, were examined. All the subjects were divided into two groups: Group without insulin resistance signs (HOMA-IR index &lt; 2.5 units) and Group with insulin resistance signs (HOMA-IR index &gt; 2.5 units). We used immunochemiluminescent and enzymatic methods, and heart rate variability was assessed using the Varikard (Russia).</p></sec><sec><title>RESULTS</title><p>RESULTS: Our research showed that 48% of all the examinees exhibited signs of insulin resistance along with an increase in the sympathetic activity in heart rate regulation. We also identified the heart rate indicators that had proved to undergo the most significant changes depending on the HOMA-IR index and the presence or absence of signs of insulin resistance: MxDMn, pNN50, SDNN, AMo50, SI, TP, HF, LF, and Body Mass.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: In general, the results obtained allow for ascertaining the high proportion of male Northerners of mature age with signs of insulin resistance. We also claim that those examinees demonstrate an autonomic imbalance and a moderate dominance of the sympathetic activity with a simultaneous decrease in activation of the parasympathetic link of autonomic nervous system and high body mass variables. At the same time, the correlations and causal associations among signs of insulin resistance, activation of the sympathetic link of autonomic nervous system, and overweight remain unclear. Apparently, all the analyzed features are likely to complement each other rather than completely exclude each other. The triad of obesity, signs of insulin resistance, and activation of the sympathetic link of autonomic nervous system is a driving factor for significant health risks.
