Вопрос влияния ожирения на организм в целом и в зависимости от выраженности метаболических нарушений остается неоднозначным. В России имеется малое количество данных о распространенности метаболических фенотипов среди населения, основанных на результатах эпидемиологических исследований.</p></sec><sec><title>Цель</title><p>Цель. Оценка распространенности метаболических фенотипов у жителей Арктической зоны Российской Федерации (на примере г. Архангельска).</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено поперечное исследование случайной выборки населения г. Архангельска (n=2380) в возрасте 35–69 лет, сформированной в рамках популяционного исследования сердечно-сосудистых заболеваний «Узнай свое сердце» (далее — УСС). Участники были разделены на группы по четырем метаболическим фенотипам в зависимости от наличия ожирения (ИМТ≥30 кг/м2) и метаболического синдрома (AHA/NHBLI): фенотип 1 — без ожирения и метаболических нарушений, фенотип 2 — без ожирения с метаболическими нарушениями, фенотип 3 — с ожирением без метаболических нарушений, фенотип 4 — с ожирением и метаболическими нарушениями.</p></sec><sec><title>Результаты</title><p>Результаты. В данное исследование включены 2352 участника УСС с наличием данных по анализируемым переменным, среди них 982 (41,8%) мужчины и 1370 (58,3%) женщин. Средний возраст составил 53,9 (SD 9,7) года. Участников разделили на группы по метаболическим фенотипам следующим образом: фенотип 1 имели 1167 (49,6%) человек, фенотип 2 — 489 (20,8%), фенотип 3 — 248 (10,5%), фенотип 4 — 448 (19,1%) человек. У мужчин вторым по частоте встречаемости после фенотипа 1 был фенотип 2, а у женщин вторую позицию разделили фенотипы 2 и 4, встречающиеся с примерно одинаковой частотой. Самым частым компонентом метаболического синдрома явилась «артериальная гипертензия», которая встречалась у 68–96% мужчин и 38–94% женщин в исследовании с разными фенотипами. Доли фенотипов с метаболическими нарушениями увеличивались с возрастом.</p></sec><sec><title>Заключение</title><p>Заключение. При обследовании случайной выборки жителей г. Архангельска в рамках концепции метаболических фенотипов половина участников не имели ожирения и метаболического синдрома. По 20% участников имели метаболические нарушения в сочетании с ожирением или без такового. Лишь 10% лиц имели «метаболически здоровое» ожирение. При исключении лиц без ожирения и метаболического синдрома среди мужчин наиболее часто встречался фенотип, характеризующийся наличием метаболических нарушений при отсутствии ожирения, среди женщин — фенотипы с наличием метаболических нарушений на фоне ожирения или без него. Самым частым компонентом метаболического синдрома явилась «артериальная гипертензия». Отмечалось накопление метаболических нарушений с возрастом.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Influence of obesity on the body at whole and with regard to metabolic changes is still unclear. In Russia there are a few data about prevalence of metabolic phenotypes among population based on epidemiological data.</p></sec><sec><title>AIM</title><p>AIM: to assess the prevalence of metabolic phenotypes among citizens of Arctic area of the Russian Federation (in the Arkhangelsk city setting).</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: a cross-sectional study was conducted using a random sample of Arkhangelsk citizens (n=2380) 35–69 years old, which was obtained within a population study of cardiovascular diseases («Know your heart» (KYH)). The participants were divided into metabolic phenotypes according to the presence of obesity (BMI≥30 kg/m2) and metabolic syndrome (AHA/NHBLI): phenotype 1 — metabolically healthy normal weight, phenotype 2 — metabolically unhealthy normal weight, phenotype 3 — metabolically healthy obesity, phenotype 4 — metabolically unhealthy obesity.</p></sec><sec><title>RESULTS</title><p>RESULTS: 2352 participants of KYH were included in the study, 982 (41,8%) men and 1370 (58,3%) women. Mean age was 53,9 (SD 9,7) years. The distribution of participants by metabolic phenotypes was as follows: 1167 (49,6%) persons had phenotype 1, 489 (20,8%) — phenotype 2, 248 (10,5%) — phenotype 3, 448 (19,1%) — phenotype 4. In men, the second common after the first phenotype was phenotype 2, while in women, the second position was shared by the 2nd and 4th phenotypes, which had approximately the same frequency. «Arterial hypertension» was the most prevalent component of metabolic syndrome and seen in 68–96% men and 38–94% women in the study with different phenotypes. The proportions of phenotypes with metabolic disorders increased with age.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: in a study of a random population sample within the framework of the concept of metabolic phenotypes, a half of the participants had no obesity and metabolic syndrome. Proportions of participants with metabolic disorders with and without obesity was 20% each. Only 10% of participants had «metabolically healthy» obesity. If excluding individuals without obesity and metabolic syndrome, the phenotype characterized by metabolic disorders in the absence of obesity was the most common among men. Phenotypes with metabolic disorders on the background of obesity or without obesity were equally common among women. The most common component of metabolic syndrome was «arterial hypertension».
