Неалкогольная жировая болезнь печени (НАЖБП) является наиболее распространенным хроническим заболеванием печени в мире, которое включает в себя изменения от печеночного стеатоза и неалкогольного стеатогепатита до фиброза и цирроза. В отдельных исследованиях проводился анализ взаимосвязей между шкалой FINDRISC и стеатозом печени и был сделан вывод о том, что данная анкета может быть использована в рамках популяционного скрининга для выявления лиц, подверженных риску развития стеатоза печени. Анализ проведенных нами литературных данных не выявил клинических исследований, посвященных вопросам использования и оценки эффективности шкалы FINDRISC в отношении скрининга фиброза печени.</p></sec><sec><title>Цель</title><p>Цель. Оценить диагностическую ценность шкалы FINDRISC в отношении выявления фиброза печени.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включали пациентов 40-60 лет из неорганизованной популяции амбулаторных пациентов. Выборка пациентов формировались произвольно согласно критериям включения и невключения. Использовался опросник FINDRISC. Всем пациентам проводилось трансабдоминальное ультразвуковое исследование печени и транзиентная эластометрия печени.</p></sec><sec><title>Результаты</title><p>Результаты. По результатам опроса по шкале FINDRISC увеличение риска развития СД 2 типа (≥7 баллов) было выявлено у 68% пациентов. Стеатоз печени был диагностирован у 41% пациентов. Медиана значений модуля эластичности печени составляла 4,50 (4,00; 5,25) кПа. При этом значения модуля эластичности печени ≥5,9 кПа были зарегистрированы у 11 (11,0%) пациентов. При анализе массива значений чувствительности и специфичности с помощью ROC-кривой было выявлено, что для шкалы FINDRISC максимальное значение LR+ и минимальное значение LR- наблюдались при количестве баллов по указанной шкале более 10. При данном отрезном значении шкала FINDRISC имела чувствительность 81,8% и специфичность 61,8% в отношении выявления фиброза печени (модуль эластичности печени ≥5,9кПа). При этом указанная шкала характеризовалась хорошей диагностической ценностью (AUC 0,699; 95% ДИ 0,530-0,815).</p></sec><sec><title>Заключение</title><p>Заключение. В неогранизованной выборке пациентов в возрасте 40-60 лет шкала FINDRISC может служить инструментом диагностики фиброза и стеатоза печени. Сумма баллов по шкале FINDRISC &gt;10 позволяла диагностировать фиброз печени (модуль эластичности печени ≥5,9кПа) с чувствительностью 81,8% и специфичностью 61,8%. Вероятность отсутствия фиброза печени при значениях шкалы FINDRISC &lt;10 составила 96,5%.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: Nonalcoholic fatty liver disease (NAFLD) is the most common chronic liver disease in the world, which includes changes from hepatic steatosis and nonalcoholic steatohepatitis to fibrosis and cirrhosis. Attempts to find noninvasive markers of liver fibrosis have led to a variety of scales, diagnostic algorithms, and imaging techniques. Individual studies have analyzed the relationship between the FINDRISC scale and hepatic steatosis and concluded that this questionnaire can be used as part of population screening to identify individuals at risk for hepatic steatosis. However, our review of the literature did not reveal any clinical studies on the use and effectiveness of the FINDRISC in liver fibrosis screening.</p></sec><sec><title>AIM</title><p>AIM: To evaluate diagnostic value of FINDRISC for liver fibrosis detection.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: The study enrolled patients aged 40–60 years from unorganized outpatient population. The sample of patients was formed randomly according to the inclusion and noninclusion criteria. All patients were assessed with standard anthropometric parameters. The FINDRISC questionnaire was used. All patients underwent transabdominal ultrasound examination of the liver and transient liver elastometry. The degree of steatosis was evaluated using Hamaguchi ultrasound scale.</p></sec><sec><title> RESULTS</title><p> RESULTS: The study included 100 patients. An increased risk of type 2 DM (≥7 points) was detected in 68% of patients using the FINDRISC scale. Liver steatosis was diagnosed in 41% of patients. Median values of hepatic elastic modulus by transient elastometry were 4.50 (4.00; 5.25) kPa. At the same time, liver elasticity modulus values ≥5.9 kPa were registered in 11 (11.0%) patients. When analyzing the array of sensitivity and specificity values using the ROC-curve, it was found that for the FINDRISC scale the maximum LR+ and the minimum LRvalues were observed when the number of points on the indicated scale exceeded 10. At this cutoff, the FINDRISC scale had a sensitivity of 81.8% and specificity of 61.8% for detecting liver fibrosis (liver modulus of elasticity ≥5.9 kPa). The scale was of good diagnostic value (AUC 0.699; 95% CI 0.530–0.815).</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: In an unorganized sample of patients aged 40–60 years the FINDRISC can serve as a diagnostic tool for liver fibrosis and steatosis. Sum of FINDRISC scores &gt;10 allowed to diagnose liver fibrosis (liver elastic modulus ≥5.9kPa) with sensitivity 81.8% and specificity 61.8%.
