Исследования, посвященные связи ожирения и функциональных расстройств кишечника, выполненные в разных возрастных группах, предоставляют противоречивые результаты. Остается неясным, в каком возрасте и под влиянием каких факторов происходит переход от свойственной детям с ожирением склонности к запорам к диарее, характеризующей взрослых пациентов с этим заболеванием.</p></sec><sec><title>Цель</title><p>Цель. Установить факторы, связанные с консистенцией стула как суррогатным маркером транзита по толстой кишке у подростков с ожирением.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведено одноцентровое обсервационное поперечное исследование. Выборка формировалась сплошным методом из числа подростков 11–17 лет с ожирением. Социально-демографические характеристики, особенности образа жизни и питания оценивали с помощью анкетирования. Симптомы со стороны кишечника — с помощью анкетирования и опроса. Консистенцию кала оценивали при помощи Бристольской шкалы форм стула. Определяли содержание в сыворотке крови аланинаминотрансферазы, аспартатаминотрансферазы, билирубина, холестерина, глюкозы, гликированного гемоглобина, лептина, инсулина. О наличии инсулинорезистентности судили по величине индекса HOMA-IR.</p></sec><sec><title>Результаты</title><p>Результаты. В исследование были включены 110 подростков с ожирением. Из них 69,1% имели патологическую консистенцию кала, с превалированием форм, характеризующих замедленный транзит по толстой кишке (49,1%). О преобладании неоформленного стула сообщили 5,5% пациентов. Сочетание разных форм стула (неустойчивый стул) описали 14,5% подростков.</p><p>Твердый стул был свойственен подросткам из неполных семей и подросткам, редко употребляющим молочные продукты. Наличие неоформленного стула ассоциировалось с инсулинорезистентностью.</p></sec><sec><title>Заключение</title><p>Заключение. Большинство подростков с ожирением, не предъявляющих активных абдоминальных жалоб, имеют признаки нарушения транзита по толстой кишке, что требует направленного диагностического поиска со стороны врача. При запорах оправданной является коррекция диеты. Необходимо проведение исследований, направленных на глубокое изучение кишечной микробиоты как возможного связующего звена между ожирением и диареей.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: Studies of the relationship between obesity and functional bowel disorders, carried out in different age groups, provide conflicting results. It remains unclear what factors are responsible for the transition from a tendency to constipation in children to a tendency to diarrhea in adults with obesity.</p></sec><sec><title>Aim</title><p>Aim: To establish factors related to stool consistency as a surrogate marker of colon transit in adolescents with obesity.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: A single-center observational cross-sectional study was carried out. We consecutively recruited adolescents, aged 11–17 years with obesity. Socio-demographic characteristics, lifestyle and nutritional characteristics were assessed using questionnaires. Bowel symptoms were assessed using questionnaires and interviews. Stool consistency was assessed using the Bristol Stool Form Scale. Serum concentrations of ALT, AST, bilirubin, cholesterol, glucose, HbA1c, leptin, and insulin were determined. The HOMA-IR index was used to determine insulin resistance.</p></sec><sec><title>Results</title><p>Results: One hundred and ten adolescents with obesity were enrolled in the study. Of these, 69.1% had a pathological consistency of feces, with the prevalence of forms characterizing delayed gut transit (49.1%). The predominance of loose stools was reported by 5.5% of patients. The combination of different forms of stool (unstable stool) was described by 14.5% of adolescents. Hard stools were common among adolescents from single-parent families and adolescents who rarely consume dairy products. The presence of loose stools has been associated with insulin resistance.</p></sec><sec><title>Conclusion</title><p>Conclusion: Most obese adolescents who do not have abdominal complaints have colon transit disorders. Medical professionals should actively ask these patients about stool frequency and properties. With constipation, dietary correction is justified.
