По литературным данным, хронический послеоперационный гипопаратиреоз составляет до 75%, в остальных случаях заболевание имеет генетическую, аутоиммунную или идиопатическую природу.</p><p>На сегодняшний день хронический гипопаратиреоз остается последним эндокринным заболеванием, для которого использование гормональной терапии не нашло широкого применения. Достижение целевых показателей фосфорно-кальциевого обмена — важный критерий компенсации заболевания, необходимый для профилактики краткосрочных и долгосрочных осложнений. Стандартная терапия активными метаболитами/аналогами витамина D (альфакальцидол, кальцитриол) и препаратами кальция не всегда позволяет достичь компенсации заболевания. Некоторые пациенты подвержены колебаниям кальция крови и продолжают испытывать симптомы гипокальциемии, несмотря на прием больших доз препаратов кальция и витамина D.</p><p>Для контроля над заболеванием используется измерение биохимических показателей кальция и фосфора сыворотки крови в ранние утренние часы, однако не всегда данные показатели отражают истинную картину.</p><p>В представленных клинических случаях описаны чередования эпизодов как гипер- так и гипокальциемии в пределах одного дня у пациенток с хроническим послеоперационным гипопаратиреозом на фоне стандартного лечения.</p></abstract><trans-abstract xml:lang="en"><p>Hypoparathyroidism is a rare endocrine disorder characterized by hypocalcemia and hyperphosphatemia, due to absent or inappropriately low serum parathyroid hormone (PTH) levels. The chronic postoperative hypoparathyroidism accounts approximately 75% of patients; the genetic, autoimmune or idiopathic forms are significantly less common. Today, chronic hypoparathyroidism remains the last major endocrine deficiency, for which hormonal replacement therapy has not found widespread use. Achieving the target levels of phosphorus-calcium metabolism is an important factor of the disease control, required for the prevention of short- and long-term complications. The standard therapy with active metabolites/analogues of vitamin D (­alfacalcidol, calcitriol) and calcium supplements does not always allow to achieve the target treatment goals.
