Предметом дискуссии является вопрос относительно разделительной точки, определяющей достаточные уровни витамина D для здоровья костной ткани. При определении адекватного референсного интервала витамина D исследователи в основном руководствуются результатами исследований, где определен уровень 25(OH)D, при котором уровень паратиреоидного гормона (ПТГ) снижается и достигает плато.</p></sec><sec><title>Цель</title><p>Цель. Установить «точку отсечения» витамина D методом подавления избыточной секреции ПТГ.</p></sec><sec><title>Методы</title><p>Методы. Проведено обсервационное одноцентровое одномоментное, выборочное неконтролируемое исследование поиска уровня витамина D по влиянию на секрецию ПТГ у жителей Тюменского региона (n=176). У всех отобранных участников исследования определялись уровни 25(ОН)D и ПТГ в сыворотке крови. Расчет «точки отсечения» производился с использованием метода поиска изменения корреляционной зависимости ПТГ от уровня витамина D, с последующей проверкой полученных данных с помощью ROC-анализа.</p></sec><sec><title>Результаты</title><p>Результаты. Проведенный математический анализ зависимости 25(OH)D и ПТГ показал значение «точки отсечения» витамина D, равное 23,6 нг/мл.</p></sec><sec><title>Заключение</title><p>Заключение. «Точка отсечения», равная 23,6 нг/мл, является оптимальной для подавления избыточной секреции ПТГ. Полученные данные могут явиться стимулом для дальнейшей отработки «точки отсечения» витамина D для российской популяции, а также использоваться для уточнения классификации дефицита, недостаточности и оптимальных уровней витамина D для населения Российской Федерации.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>BACKGROUND</title><p>BACKGROUND: The subject of discussion is the issue of the separation point, which determines sufficient levels of vitamin D for bone health. When determining the adequate reference range of vitamin D, researchers are mainly guided by the results of research, where the level of 25(OH)D is determined, at which the PTH level decreases and reaches a plateau.</p></sec><sec><title>AIM</title><p>AIM: To establish the «cut-off point» of vitamin D by suppressing excessive secretion of PTH.</p></sec><sec><title>MATERIALS AND METHODS</title><p>MATERIALS AND METHODS: Observational, single-site, single-stage, selective, uncontrolled study of the search for vitamin D levels by the effect on PTH secretion in residents of Tyumen region was conducted (n = 176). All selected study participants determined the level of 25(OH)D and PTH in serum. The calculation of the «cut-off point» was carried out using the method of searching for changes in the correlation dependence of PTH on the level of vitamin D, followed by verification of the data obtained using ROC analysis.</p></sec><sec><title>RESULTS</title><p>RESULTS: A mathematical analysis of the dependence of 25(OH)D and PTH showed the “cut-off point” of vitamin D, equal to – 23.6 ng/ml.</p></sec><sec><title>CONCLUSION</title><p>CONCLUSION: The “cut-off point” of 23.6 ng/ml is optimal for suppressing excessive PTH secretion.
